The Beacon Project

The Beacon Project connects people being released from local hospitals with community supports and services such as housing, food and transportation. Acting on a growing body of research that connects health outcomes with economic and social conditions, patients can get better more quickly with an activated care plan that includes more than just medical services. As part of the Beacon Project, 232-HELP's Community Care Navigators in collaboration with local hospitals will screen patients and connect those in need with food, housing and more.
